Termination of placement

Form to fill out if you want to terminate the placement. Rules for terminating a place apply from the day the contract is signed. Notice must be given in writing and submitted to the manager of the day care facility. There is a mutual one month period of notice applying from the 1st in the month or the 15th. Fees must be paid until the end of June should notice of terminating a place be received after the 1 April.
